Home Health Aide Job Description

At Horizon Home Care & Hospice, we are seeking a compassionate and skilled Home Health Aide to join our team. As a Home Health Aide, you will play a vital role in providing direct assistance with activities of personal care, maintenance of patient health, and facilitation of treatment for patients in their homes.

Key Responsibilities:

  • Provide assistance with daily living activities, such as bathing, dressing, transfers, and toileting, under the supervision of a Registered Nurse or Therapist.
  • Review care plans with patients and notify Case Managers of any issues or concerns.
  • Explain care being provided to patients and involve them in their care.
  • Complete mobile care upon arrival and departure from patients' homes, including tasks completed in accordance with the care plan.
  • Work cooperatively with patients, care providers, and family members to ensure patient needs are met.
