Key Responsibilities:

  • Assist patients with daily living activities, such as bathing, dressing, and toileting
  • Provide support with mobility and transfers
  • Assist with meal preparation and light housekeeping
  • Monitor and report any changes in patient condition to the Case Manager
  • Work collaboratively with patients, families, and healthcare providers to ensure seamless care
