Home Health Aide Job Description

At Horizon Home Care & Hospice, we are seeking a compassionate and skilled Home Health Aide to join our interdisciplinary team. As a Home Health Aide, you will provide personalized care and support to patients in the comfort of their own homes.

Key Responsibilities:

  • Assist patients with daily living activities, such as bathing, dressing, and toileting, under the supervision of a Registered Nurse or Therapist.
  • Review care plans and notify Case Managers of any issues or concerns.
  • Provide assistance with mobility, transfers, and range of motion exercises.
  • Engage patients in their care and involve them in decision-making processes.
  • Complete mobile care upon arrival and departure from patients' homes, including tasks outlined in the care plan.
