Key Responsibilities
  1. Develop and implement effective care plans for clients, ensuring seamless access to services.
  2. Collaborate with multidisciplinary teams to address complex client needs.
  3. Provide linguistically appropriate services to clients, leveraging bilingual skills.
Qualifications
  • Bachelor's Degree in a related field.
  • Experience working with vulnerable populations, preferably in a healthcare setting.
  • Strong understanding of community resources and services.

NYPCC offers a competitive salary range of $45,000 - $50,000 per year, plus comprehensive benefits, including medical, dental, vision, and more.
