About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at BoldAge PACE. As a key member of our healthcare team, you will play a vital role in delivering high-quality care to our elderly participants.

Key Responsibilities
  • Assess and plan individualized interventions in accordance with the care plan.
  • Administer medications and implement primary care orders.
  • Evaluate participant complaints and provide education on care plans, nursing treatments, and medications.
  • Communicate participant conditions to primary caregivers, physicians, and the multidisciplinary team.
Requirements
  • Graduate of an accredited school of nursing.
  • Current LPN license in good standing to practice in the required state.
  • IV Certification preferred.
  • Experience with paperless documentation preferred.
  • 1 year of experience working with a frail or elderly population preferred.
  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check.
