Social Work Professional ARRMC

We are seeking a skilled Social Work Professional to join our team at Asante. As a key member of our interdisciplinary team, you will provide essential services to patients and their families, enhancing their social and emotional functioning and improving their quality of life.
Key Responsibilities- Provide social work services to patients and their families, addressing social and emotional problems and promoting optimal medical care utilization.
- Collaborate with healthcare teams to develop and implement patient-centered care plans.
- Participate in interdisciplinary and interagency collaboration and consultation to ensure comprehensive patient care.
- Conduct assessments, develop treatment plans, and provide counseling and education to patients and their families.
- Document patient interactions and progress in accordance with Asante's policies and procedures.
- Master's degree in Social Work (MSW) from an accredited program.
- 2 years of social work practice experience or student practicum.
- Valid Driver's License with proof of safe driving record for the past ten years and proof of auto insurance (if required).
- Preferred licensure: LCSW or LMSW.
