Job Summary

We are seeking a skilled Social Work Professional to join our team at Asante. As a key member of our interdisciplinary team, you will provide essential services to patients and their families, enhancing their social and emotional well-being and promoting quality of life.

Key Responsibilities
  • Provide social work services to patients and their families, addressing social and emotional challenges and promoting optimal medical care utilization.
  • Collaborate with healthcare teams to develop and implement patient-centered care plans.
  • Participate in interdisciplinary and interagency collaboration and consultation to ensure comprehensive patient care.
  • Conduct assessments, develop treatment plans, and provide counseling and therapy services as needed.
  • Document patient progress and maintain accurate records.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • 2 years of social work practice experience or student practicum.
  • Valid Driver's License with proof of safe driving record and auto insurance (if required).
  • Preferred licensure: LCSW or LMSW.
