Job Title: Nurse Practitioner - Complex Care Specialist

Job Summary:

We're seeking a highly skilled and compassionate Nurse Practitioner to join our team in Rockingham and Hillsboro Counties, NH. As a member of our interdisciplinary team, you'll work closely with patients, families, and healthcare professionals to deliver high-quality, patient-centered care.

Key Responsibilities:
  • Perform comprehensive assessments and develop individualized care plans for patients with complex medical needs.
  • Collaborate with healthcare professionals to coordinate care and ensure seamless transitions between settings.
  • Prescribe medications and order diagnostic tests as needed.
  • Provide education and support to patients and families to promote health and wellness.
  • Participate in ongoing quality improvement initiatives to enhance patient outcomes and care delivery.
Requirements:
  • Completion of a Nurse Practitioner accredited program.
  • Current state RN/NP license.
  • Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
  • Access to reliable transportation.
Preferred Qualifications:
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
  • Knowledge of long-term care, community resources, and cost-effective alternatives.
  • Knowledge of CPT, ICD-10, and HCPC codes.
  • Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
