As a RN Case Manager, you will be responsible for supervising the personal care, companion and skilled nursing services provided by our direct care staff. Your primary goal will be to ensure the quality of care for our clients in assigned areas.

Key Responsibilities:

  • Supervise direct care staff to ensure quality of care and adherence to company policies
  • Conduct supervisory visits of clients, documenting visits and orientation and training of caregivers as needed
  • Perform skilled tasks as required, including LPNs as defined by Company policy
  • Ensure client care issues, changes and/or updates are documented and communicated to branch staff, direct care staff, case management and family
  • Review HRST and other documentation as required for DD cases, meeting with branch staff, direct care staff, family and DD support team as necessary
  • Assess and recommend solutions in areas of risk for clients, staff and organization medical practice
  • Provide PRN visits as necessary per Company policy and/or as directed by supervisor
  • Ensure the compliant maintenance of TB testing supplies and proper disposal of syringes

Requirements:

  • At least 24 months of prior nursing experience, preferably within a multiple location environment
  • Knowledge of health administration regulations and standards, health information management, and Medicaid, Waiver and Home health service lines
  • Advanced knowledge of Microsoft Office Suite and aptitude to learn new applications
