Job Summary

As a highly skilled Care Coordinator Specialist at Aspire Behavioral Health & DD Services, you will play a vital role in providing comprehensive case management services to patients/clients/youths and their families. With a strong focus on customer satisfaction and youth outcomes, this position requires a compassionate and organized individual who can effectively coordinate care plans, provide crisis intervention, and facilitate community resources.

Responsibilities

• Conduct patient/client/youth intakes, participate in treatment team planning, and perform crisis intervention
• Assist in dealing with personal and social problems
• Provide behavioral and emotional support to youths and families, serving as a liaison for social services
• Perform case management duties, identifying and gaining access to required services and supports
• Coordinate a set of interrelated activities for identifying, planning, budgeting, documenting, coordinating, securing, and reviewing the delivery and outcome of appropriate services for individuals through a wraparound approach

Requirements

• High school diploma or GED, with two years' experience in a social services-related position, OR a Bachelor's degree in a social services-related field, OR one year at the lower level or position equivalent
• Strong knowledge of agency policies and procedures, ability to coordinate and organize delivery services, and monitor youth's progress toward meeting established goals
• Excellent communication and interpersonal skills, with the ability to develop and implement life skills workshops and programs in behavior management, youth services, community, and social services
• Ability to work collaboratively with co-workers, other contacts, and families/caregivers/legal guardians to provide consistent, high-quality, youth-focused services
• Estimated salary: $55,000 - $65,000 per annum, depending on location and experience
