Hospice Care Specialist

About the Role:
We are seeking a dedicated and compassionate Registered Nurse to join our Hospice team as a Hospice RN. As a Hospice RN, you will provide high-quality patient care and support to patients and their families in their homes, hospitals, and other care settings.
Key Responsibilities:
- Provide direct patient care services to hospice patients and supportive care to the patient's family.
- Act as primary liaison between physician, patient/family and hospice team.
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
- Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff to provide comfort, peace, and dignity.
Requirements:
- RN license in the state of requested employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
- Current driver's license and automobile insurance.
- Current CPR certification.
