At Falcon Healthcare, we're dedicated to providing exceptional care to our patients in the comfort of their own homes. As a Licensed Vocational Nurse (LVN) on our hospice team, you'll play a vital role in delivering compassionate and personalized care to those in need.

As an LVN with Falcon Healthcare, you'll have the opportunity to make a meaningful difference in the lives of our patients and their families. You'll work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs and goals of each patient.

Responsibilities of the LVN role include:

  • Following a designated care plan in accordance with the patient's needs
  • Performing assigned duties, including administration of medication, wound care, treatments, and procedures
  • Following up with, executing, and properly documenting doctors' orders
  • Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided

To be successful in this role, you'll need to possess a current Practical/Vocational Nursing License in the state where you intend to work, as well as a minimum of 1 year of nursing experience. Home Health or Hospice experience is a plus, and you'll need to be certified in CPR. Strong organizational and communication skills are essential, and you'll need to provide proof of current auto insurance and Covid vaccine or medical exemption.
