About the Role

We are seeking a skilled Hospice Transition Liaison RN to join our team at Enhabit Home Health & Hospice. As a key member of our team, you will play a vital role in ensuring seamless transitions for our patients and their families.

Key Responsibilities
  • Represent Enhabit Home Health & Hospice in internal transitional care activities and professional contacts with patients, families, physicians, hospitals, and facilities.
  • Develop patient-centered transition plans that integrate clinical guidelines, tools, and metrics to promote quality and efficiency in care delivery.
  • Manage public and internal education relative to hospice services available and payor sources, especially the Care Connections program.
Requirements
  • Graduate of an approved school of nursing.
  • Licensed in the state where they currently practice.
  • At least two years of clinical experience.
  • Demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing home health and hospice operations.
  • Basic technology skills, including operation of a mobile device.
Preferred Qualifications
  • At least three years of clinical experience.
  • Previous experience in and knowledge of home health and hospice practices.
  • Demonstrated experience and understanding of customer and market dynamics, as well as transitional care requirements and best practices within home health or hospice.
