Mental Health Professional with Assertive Community Treatment Team

As a Mental Health Professional on our Assertive Community Treatment Team, you will play a vital role in delivering high-quality services to individuals with severe and persistent mental illness.
Job DescriptionWe are seeking a skilled Mental Health Professional to join our ACT team, where you will work closely with medical, behavioral health, and rehabilitation professionals to provide holistic care to our clients. Your primary responsibilities will include:
- Assessing and tracking stages of change readiness and treatment plans for individuals assigned to your caseload.
- Facilitating the Person-Centered Planning process to ensure that treatment approaches align with each individual's unique needs and goals.
- Utilizing cognitive-behavioral approaches and relapse prevention techniques to support individuals in later stages of change readiness.

Requirements- Bachelor's degree in Human Services, Social Work, Special Education, or a related field.
- At least two years of professional experience working with adult mental health populations.
- Valid driver's license, reliable transportation, and auto insurance coverage.
- Ability to perform tasks on the computer, including proficiency in Microsoft 360, Excel, and Word.
We offer a competitive salary range of $55,000 - $65,000 per year, based on experience, plus a comprehensive benefits package that includes:
