About the Role

Enhabit Home Health & Hospice is seeking a skilled Clinical Team Leader to oversee the coordination of home health services to clients. As a key member of our team, you will ensure the delivery of high-quality patient care while promoting a collaborative and innovative work environment.


Responsibilities

The Clinical Team Leader will be responsible for:

  • Coordinating home health services to ensure seamless patient care
  • Reviewing and approving patient information submitted by licensed professionals
  • Providing oversight of patient care and assisting with case conferences
  • Assisting with day-to-day office and staff management related to patient care
  • Contributing to the overall company success of providing A Better Way To Care by effectively facilitating relationships between physicians, referral sources, patients, caregivers, and employees

Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing and licensure in the state of employment
  • A minimum of two years of nursing experience and one year of experience in home health
  • A good understanding of federal, state, and local laws and regulations governing home health agencies
  • Skilled competency with technology, including general office equipment and EMR platforms
  • A valid state driver's license and automobile liability insurance
