Role Responsibilities:

Reporting directly to the Director of Geriatric Services, the Clinical Liaison for Geriatric Services plays a crucial role in facilitating communication among partner facilities, families, and patients regarding available program services and support. This position involves advocating for patients by providing outreach, education, assessment, and treatment assistance to enhance access to care and foster collaboration among patients, families, the Care at Home team, and congregate living staff.

Key Responsibilities Include:

  • Traveling to patient residences and care facilities within the Signature Health service area.
  • Conducting age-appropriate face-to-face assessments for new patients in their community or home settings.
  • Engaging with clinical and administrative staff at partner facilities to ensure effective communication and collaboration.
  • Providing advocacy, education, and support to patients and families regarding issues related to aging and available community resources.
  • Coordinating with Care at Home clinical and administrative teams regarding specific patient needs or facility-related concerns.
  • Facilitating community education initiatives for congregate living facilities and other organizations.
  • Building relationships with stakeholders in the mental health and aging sectors to promote program growth.
  • Participating in networking and outreach for business development.
  • Maintaining patient documentation in compliance with relevant regulatory bodies such as Medicare, Medicaid, OMHAS, and CARF, as well as adhering to professional standards.
  • Providing telehealth services as required by Signature Health.
  • Completing compliance training as mandated by Signature Health.

Qualifications:

Education & Experience:

  • A Master's Degree is required.
  • A valid, unencumbered independent license (LISW, LPCC, or LIMFT) in the state of Ohio is required.
  • 1-2 years of experience in assessment or counseling is preferred.
  • 1-2 years of experience working with the aging population is preferred.
  • Proficiency in Microsoft Office and Electronic Health Records is essential, along with knowledge of word processing software.

Work Environment:

  • Work is typically conducted in an office or clinical environment, with additional responsibilities in home-based settings.
  • While standard hours are generally followed, flexibility for evening and extended hours may be necessary.
  • Job duties require periods of sitting, standing, and using a telephone and computer.
  • Hearing, speaking, and visual acuity must be adequate to perform job functions effectively.
  • Physical demands may include occasional lifting of items weighing up to 15 pounds.
  • Exposure to bloodborne pathogens may occur while performing job duties.
  • Regular activities will involve bending, reaching, and repetitive movements, along with some lifting and pushing.
  • Dexterity is required to operate a PC and other office equipment.
