Position Overview:

Hybrid Work Setting Required

The Integrated Care Management Specialist will be responsible for coordinating care for eligible members, fostering a collaborative environment among members, healthcare providers, community organizations, and staff to guarantee that members receive prompt and cohesive access to necessary services. This role involves executing health action plans and care strategies.

Essential Qualifications:

- A Bachelor's degree coupled with a minimum of three years of experience in case management, including conducting assessments and home/community visits.

- Strong communication and time management abilities.

- Exceptional interpersonal, problem-solving, and organizational skills.

- Demonstrated cultural sensitivity towards individuals from diverse ethnic backgrounds and socioeconomic statuses.

- Proficient in Microsoft Word and Outlook; basic knowledge of Microsoft Excel and PowerPoint.

- A valid driver's license and access to a vehicle with current registration and insurance are mandatory.

- Ability to safely operate an insured vehicle for travel to various locations as required for job responsibilities.

Key Responsibilities:

- Oversee members assessed by various case management teams to implement their health action plans (HAP) and ensure timely service delivery. This may involve initiating processes, contacting providers, and following up on necessary requests.

- Review Health Action Plans/Care Plans to ensure compliance with relevant requirements, utilizing various communication methods including in-person meetings, phone calls, emails, and video conferencing.

- Manage patient care by cultivating effective relationships among providers, members, and staff to facilitate timely access to appropriate medical services.

- Maintain accurate documentation in electronic record systems, adhering to industry standards and requirements.

- Advocate for and assist members in navigating the healthcare system and accessing HMSA services.

- Participate in mandatory training sessions and apply learned skills in daily operations.

- Carry out additional responsibilities and tasks as assigned.
