About the Role:

  • Ensure the overall well-being of our residents through proper assessment, planning, and implementation of individualized care plans.
  • Work collaboratively with the Resident Care Director (RCD) to conduct resident evaluations and gather information at various stages, including move-in, 30 days post-move-in, semi-annually, and upon any change in status.
  • Conduct monthly wellness visits for all residents, monitoring and following up on any falls, accidents, or incidents, documenting and reporting as required.
  • Maintain open communication with residents' families and responsible parties to ensure medical needs are being met.
  • Contact residents' attending physicians as necessary and/or upon family request.

Responsibilities:

  1. Identify ongoing needs and services of residents to promote high-quality resident services.
  2. Ensure proper follow-through and participate in evaluation and documentation for residents with changes in healthcare services and status.
  3. Assist RCD in conducting resident evaluations and gathering information at designated intervals.
  4. Conduct monthly wellness visits for all residents.
  5. Monitor and follow up on residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
  6. Document all pertinent information in the resident wellness file.
  7. Maintain communication with residents' families and responsible parties to ensure medical needs are being met.
  8. Contact residents' attending physicians as necessary and/or upon family request.
  9. Ensure weights and vital signs are obtained monthly for each resident.
  10. Assist in maintaining wellness files according to Sunrise policies and federal/state regulations.
  11. Maintain medical supplies and emergency kits for the community.
  12. Provide clinical support and assistance to community team members as needed.

Requirements:

  • Graduate of an approved college/school of nursing.
  • Maintains a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrates knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
  • Demonstrates good assessment skills.
  • Ability to handle multiple priorities.
  • Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
  • Competent in organizational and time management skills.
  • Demonstrates good judgment, problem-solving, and decision-making skills.
  • Demonstrates proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), and Sunrise applications with the ability to learn new applications.
