Senior Nurse Care Coordinator

About the Role:
- Ensure the overall well-being of our residents through proper assessment, planning, and implementation of individualized care plans.
- Work collaboratively with the Resident Care Director (RCD) to conduct resident evaluations and gather information at various stages, including move-in, 30 days post-move-in, semi-annually, and upon any change in status.
- Conduct monthly wellness visits for all residents, monitoring and following up on any falls, accidents, or incidents, documenting and reporting as required.
- Maintain open communication with residents' families and responsible parties to ensure medical needs are being met.
- Contact residents' attending physicians as necessary and/or upon family request.
Responsibilities:
- Identify ongoing needs and services of residents to promote high-quality resident services.
- Ensure proper follow-through and participate in evaluation and documentation for residents with changes in healthcare services and status.
- Assist RCD in conducting resident evaluations and gathering information at designated intervals.
- Conduct monthly wellness visits for all residents.
- Monitor and follow up on residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
- Document all pertinent information in the resident wellness file.
- Maintain communication with residents' families and responsible parties to ensure medical needs are being met.
- Contact residents' attending physicians as necessary and/or upon family request.
- Ensure weights and vital signs are obtained monthly for each resident.
- Assist in maintaining wellness files according to Sunrise policies and federal/state regulations.
- Maintain medical supplies and emergency kits for the community.
- Provide clinical support and assistance to community team members as needed.
Requirements:
- Graduate of an approved college/school of nursing.
- Maintains a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Demonstrates knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
- Demonstrates good assessment skills.
- Ability to handle multiple priorities.
- Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
- Competent in organizational and time management skills.
- Demonstrates good judgment, problem-solving, and decision-making skills.
- Demonstrates proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), and Sunrise applications with the ability to learn new applications.

- Competitive salary range: $65,000 - $80,000 per annum, depending on qualifications and location.
