Job Overview

The Clinical Care Coordinator is responsible for promoting the highest quality resident services in our community. This involves identifying ongoing needs and services of residents, ensuring proper follow-through, and participating in evaluations and documentation. The ideal candidate will possess excellent communication and organizational skills, as well as the ability to work effectively with the healthcare team.

Responsibilities:
  1. Identify and address the ongoing needs and services of residents to ensure the highest quality resident services.
  2. Ensure proper follow-through on resident evaluations and documentation.
  3. Participate in conducting resident evaluations and data gathering information at time of move-in, 30 days post-move-in, semi-annually, and with any change in status.
  4. Conduct monthly wellness visits for all residents.
  5. Monitor and follow up with residents who have experienced falls, accidents, or incidents; document and report as required and directed.
  6. Document all pertinent information in the resident wellness file.
  7. Maintain open communication with residents' family members and responsible parties to ensure medical needs are being met.
  8. Contact residents' attending physicians when necessary and/or upon family request.
  9. Ensure weights and vital signs are obtained monthly for each resident.
  10. Assist in maintaining wellness files according to Sunrise policies and federal and state regulations.
  11. Maintain medical supplies and emergency kits for the community.
  12. Provide clinical support and assistance to community team members as needed.
  13. Understand and follow infection control practices.
  14. Partner with the Resident Care Director and community team to promote infection control standards, including resident testing, training, and skills checks.
  15. Understand and practice the proper method of attending to and disposing of bloodborne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal precautions.
  16. Regularly monitor each resident's medication and treatment profile to ensure medications and treatments are administered as ordered and documented accurately.
Requirements:
  • Graduate of an approved college/school of nursing.
  • Maintain a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrate knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
  • Demonstrate good assessment skills.
  • Ability to handle multiple priorities.
  • Possess written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
  • Competent in organizational, time management skills.
  • Demonstrate good judgment, problem-solving, and decision-making skills.
  • Demonstrate proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), and Sunrise applications with the ability to learn new applications.
  • Ability to work weekends, evenings, and flexible hours, available to customers at peak service delivery days and times.
Benefits

Sunrise Senior Living offers a competitive compensation package based on geographic location, skills, experience, qualifications, and certifications/licenses. Selected candidates will receive:

  • $65,000 - $85,000 per year, depending on location and experience.
