Patient Care Coordinator Oncology Professional

We are seeking a skilled and compassionate Patient Care Coordinator to join our team at Ascension Wisconsin.
About the Role:This is a full-time position with a competitive salary of $70,000 - $90,000 per annum, depending on experience. The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work collaboratively as part of a multidisciplinary team.
About the Job:- Serve as a patient navigator throughout the continuum of care, ensuring seamless transitions between healthcare providers and services.
- Act as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers, promoting effective communication and coordination of care.
- Compile data, track outcomes, and make recommendations for process improvement, enhancing the quality of care provided to patients.
- Provide referrals and support for follow-up care for patients who are uninsured or underinsured, ensuring equitable access to healthcare services.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- BLS Provider certification preferred, with American Heart Association or American Red Cross accepted.
- Diploma from an accredited school/college of nursing or equivalent required professional licensure at time of hire.
