Job Overview:
Clinical Care Coordinators play a vital role in the healthcare industry, ensuring seamless transitions for patients throughout their care journey.

Estimated Salary: $65,000 - $85,000 per year, depending on location and experience.

Responsibilities:
  • Coordinate interdisciplinary care plans for patients from admission to discharge.
  • Develop and implement comprehensive care plans that incorporate patient preferences and values.
  • Monitor patient progress through regular assessments and evaluations.
  • Act as a liaison between healthcare providers and patients, facilitating effective communication and resource allocation.
  • Ensure accurate and timely documentation of patient records and services rendered.

Requirements:
  • Registered Nurse licensure from the Wisconsin Board of Nursing or current home state license (for multi-state/compact states).
  • BLS Provider certification (American Heart Association or American Red Cross accepted).
  • Case Manager certification preferred (Commission for Case Manager Certification).
  • Diploma from an accredited school of nursing or required professional licensure at time of hire.
