VillageCare is seeking a skilled Nurse Assessor to join our team. As a key member of our assessment team, you will conduct comprehensive assessments using the NYS Department of Health Universal Assessment System (UAS) to ensure the health and well-being of our Members.
Key Responsibilities:
- Conduct in-person and telephonic assessments to evaluate clinical, social, and individual needs
- Create tailored care plans in collaboration with Members, families, and providers
- Document assessments promptly and work closely with the UAS Nurse Reviewer to enhance the accuracy of care plans
- Participate in quality improvement initiatives to elevate service delivery and ensure Member satisfaction
Requirements:
- Minimum of an Associate's degree in nursing; BSN preferred
- Active NYS RN License and valid NYS identification
- At least two years of clinical experience, ideally in home health care or within a health plan
- Familiarity with NYS Managed Long Term Care (MLTC) and Medicaid/Medicare plans
- Proficiency in standard office software applications and electronic health records systems
