Job Summary

We are seeking a highly skilled Palliative Care Advanced Practice Provider to join our team at Trinity Health. As a key member of our palliative care team, you will be responsible for providing high-quality patient care and education to patients, families, and healthcare teams.

Key Responsibilities
  • Clinical Responsibilities:
    • Conduct comprehensive medical and psychosocial evaluations of patients and families.
    • Mentor physicians and nurses to support the primary care team.
    • Develop and implement palliative care protocols in multiple ICUs.
    • Lead discussions on care goals with ICU teams.
    • Independently follow up on patient consultations and symptom management.
    • Participate in ICU rounds and conduct consults throughout the hospital, including the ED.
  • Educational Responsibilities:
    • Develop policies, procedures, and protocols for palliative care.
    • Present in-service sessions on palliative care to healthcare teams.
    • Train students, residents, and advance practice nurse students on palliative care consult services.
    • Precept advance practice nurse students.
  • Additional Activities:
    • Regular meetings with the Palliative Medicine Medical Director.
    • Maintain palliative care continuing education.
    • Participate in nursing department committees.
    • Lead or participate in special projects as needed.
Requirements
  • Education:
    • Master's Degree in Nursing or related field.
  • Experience:
    • 1 year of experience in inpatient palliative medicine.
  • Licensure/Certifications:
    • CPR - Basic Life Support.
    • Nursing - Current Registered Nurse License - State of Georgia.
    • Nursing - National advanced practice certification in adult, geriatrics, or family practice (preferred).
