Key Responsibilities:- Deliver palliative care to patients in a variety of settings, including hospitals, clinics, and patient homes.
- Assist in the training of other team members and medical professionals.
- Conduct face-to-face encounters with patients to assess their needs and develop personalized care plans.
- Initiate palliative care consultations and develop treatment plans in collaboration with physicians.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Provide education, counseling, and referrals to patients and their families as needed.
- Document patient care and treatment plans accurately and timely.
- Graduate of an approved Nurse Practitioner program.
- Current unrestricted license to practice as a Nurse Practitioner in the state of employment.
- Certified Hospice and Palliative Care Nurse (CHPN) preferred.
- Minimum of 2 years of palliative care and/or hospice experience.
