Responsibilities:- Deliver palliative care to patients in various settings, including hospitals, clinics, and patient homes.
- Conduct face-to-face encounters with patients to assess their needs and develop personalized care plans.
- Collaborate with medical colleagues to ensure seamless patient care and communicate effectively about patient progress.
- Provide education and support to patients and their families, empowering them to make informed decisions about their care.
- Participate in interdisciplinary team meetings to discuss patient cases and develop comprehensive care plans.
- Assist in the training and education of other healthcare professionals to enhance their knowledge and skills in palliative care.
- Meet department productivity standards for patient visits and maintain accurate patient records.
- Current unrestricted license to practice as a registered professional nurse in the state of employment.
- Graduate of an approved Nurse Practitioner program.
- Current unrestricted license to practice as a Nurse Practitioner by the Board of Nursing in the state of employment.
- Current and continued CPR certification.
- Certified Hospice and Palliative Care Nurse (CHPN) preferred.
- Minimum of 2 years of palliative care and/or hospice experience required.
