  • Collaborate with medical colleagues to ensure seamless patient care and communicate effectively about patient progress.
  • Provide education and support to patients and their families, empowering them to make informed decisions about their care.
  • Participate in interdisciplinary team meetings to discuss patient cases and develop comprehensive care plans.
  • Assist in the training and education of other healthcare professionals to enhance their knowledge and skills in palliative care.
  • Meet department productivity standards for patient visits and maintain accurate patient records.
Qualifications:
  • Current unrestricted license to practice as a registered professional nurse in the state of employment.
  • Graduate of an approved Nurse Practitioner program.
  • Current unrestricted license to practice as a Nurse Practitioner by the Board of Nursing in the state of employment.
  • Current and continued CPR certification.
  • Certified Hospice and Palliative Care Nurse (CHPN) preferred.
  • Minimum of 2 years of palliative care and/or hospice experience required.
