Hospice RN Case Manager

We're seeking a compassionate and skilled Hospice RN to join our dynamic team at Inspire Hospice and Palliative Care. As a Hospice RN, you'll have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the RoleAs a Hospice RN, you'll be responsible for providing high-quality, patient-centered care to individuals in their homes, assisted living facilities, or skilled nursing facilities. Your expertise will include:
- Assessing patient needs and developing personalized care plans
- Providing emotional support and guidance to patients and their families
- Collaborating with our interdisciplinary team to ensure comprehensive care
- Documenting patient progress and maintaining accurate records

To be considered for this role, you'll need:
- A current RN license in the state of practice
- Experience in hospice or home health care
