Overview: Practical Nursing Specialist - Takes charge of comprehensive resident care within a specified unit during each shift by executing professional nursing responsibilities and ensuring the fulfillment of nurse aide tasks. Collaborates with all departments to guarantee superior resident care.

Key Responsibilities:

  • Lead and oversee the unit in partnership with the Director of Resident Services (DRS) to ensure high-quality resident care is delivered.
  • Develop nurse aide assignments as necessary to address resident needs.
  • Inform the DRS of any changes in resident conditions and any issues that arise with staff members.
  • Administer medications and treatments following established protocols and procedures.
  • Ensure the medicine cart is secured at all times when not in direct view.
  • Update resident support plans daily as required.
  • Conduct thorough assessments of residents and act in accordance with established policies and procedures.
  • Assist residents with activities of daily living (ADLs) as needed.
  • Support residents during meal times and monitor fluid intake to ensure proper nutrition.
  • Accompany physicians during resident visits as necessary to maintain continuity of care.
  • Accurately transcribe physician orders in a timely manner per policy.
  • Observe nursing care and visit residents throughout the shift to ensure that care is delivered as directed.
  • Participate in shift report exchanges to ensure continuity of care and communicate essential information to nurse aides.
  • Supervise care provided by Nurse Aides.
  • Complete incident and accident reports for residents promptly as per policy.
  • Collect samples, such as urine or sputum, from residents for necessary testing.
  • Communicate vital information with families, residents, and staff as required.
  • Ensure that adequate supplies are available on the floor for all departments.
  • Engage in staff meetings to enhance communication across all shifts and disciplines.
  • Participate in resolving employee and floor-related issues.
  • Assist in the orientation and training of new personnel.
  • Maintain a clean, safe, and organized environment for residents, staff, and visitors.
  • Document all care provided to residents during the shift accurately and before the end of the shift.
  • Chart medications immediately after administration, following policy guidelines.
  • Provide feedback to the DRS regarding nurse aide performance.
  • Uphold confidentiality at all times.
  • Attend and engage in staff meetings, in-services, mandatory training sessions, and continuing education programs.
  • Always operate within the limits of the Pennsylvania State Professional Standards Title 49 Chapter and in accordance with the scope of practice permitted by licensure.
  • Perform additional duties as assigned.

Minimum Qualifications:

  • Graduated from an accredited nursing program and possess a current Practical Nurse license in good standing from the Commonwealth of Pennsylvania; a nurse without a Pennsylvania license must have a temporary practice permit as required by the Pennsylvania State Board of Nursing.
  • Proven management, communication, and observational skills with a focus on geriatrics.
  • Proficient in computer usage, including Microsoft Word.
