Job Summary

We are seeking a skilled Licensed Practical Nurse to join our care team as a Care Team Lead at Harrison Senior Living of Chester County. As a key member of our interdisciplinary team, you will be responsible for providing high-quality resident care and ensuring the smooth operation of our nursing services.

Key Responsibilities
  • Lead and Coordinate Resident Care: Assume charge and direction of the care team on a designated unit, ensuring that resident needs are met and care is delivered in accordance with established policies and procedures.
  • Assign and Supervise Nurse Aides: Create and manage nurse aide assignments to meet the needs of residents, providing guidance and support as needed.
  • Communicate Effectively: Notify the Director of Resident Services of any changes in resident condition or issues with staff members, ensuring seamless communication and collaboration.
  • Medication Administration: Administer medications and treatments in accordance with established policies and procedures, maintaining accurate records and ensuring resident safety.
  • Medicine Cart Management: Keep the medicine cart locked and secure when not in use, maintaining a safe and organized environment.
  • Resident Support Plans: Update resident support plans daily as needed, ensuring that care is tailored to individual needs and preferences.
  • Assessment and Intervention: Appropriately assess residents and implement interventions in accordance with established policies and procedures.
  • ADL Assistance: Assist residents with activities of daily living as needed, promoting independence and dignity.
  • Meal and Fluid Management: Assist with resident meals and fluid intake, ensuring proper nutrition and hydration.
  • Accompany Physicians: Accompany physicians on resident visits as needed, ensuring continuity of care and resident comfort.
  • Transcription and Documentation: Transcribe physician orders and document resident care in a timely and accurate manner.
  • Observation and Monitoring: Observe nursing care and visit residents throughout the shift, ensuring that care is delivered as directed and resident needs are met.
  • Report and Communication: Participate in receiving report from off-going shifts and giving report to on-coming shifts, ensuring seamless communication and continuity of care.
  • Care Quality and Safety: Monitor care provided by nurse aides, ensuring that it meets established standards and policies.
  • Incident Reporting: Complete resident incident/accident reports in a timely manner, maintaining accurate records and ensuring resident safety.
  • Sample Collection: Collect samples, such as urine or sputum, from residents for testing as needed.
  • Communication and Collaboration: Communicate information as necessary with families, residents, staff, and other stakeholders, promoting a positive and supportive environment.
  • Supply Management: Ensure that appropriate and adequate supplies are available on the unit for all disciplines.
  • Staff Meetings and Training: Participate in staff meetings, in-services, and training programs, enhancing communication and collaboration between disciplines.
  • Problem Resolution: Participate in problem resolution regarding employees and other floor issues, promoting a positive and respectful work environment.
  • Orientation and Training: Participate in the orientation and training of new personnel, ensuring a smooth transition and effective onboarding.
  • Environment of Care: Maintain a clean, safe, and orderly environment for residents, staff, and visitors, promoting a positive and supportive atmosphere.
  • Documentation and Charting: Accurately document all care provided to residents during the shift and before the end of the shift, maintaining accurate records and ensuring resident safety.
  • Medication Charting: Chart medications immediately after administration, per policy.
  • Nurse Aide Performance: Provide input to the Director of Resident Services concerning nurse aide performance, promoting a positive and supportive work environment.
  • Confidentiality: Maintain confidentiality in all aspects of resident care and communication.
  • Continuing Education: Attend and participate in staff meetings, in-services, mandatory trainings/in-services, and continuing education programs, enhancing knowledge and skills.
  • Scope of Practice: Function within the limits of the Pennsylvania State Professional Standards Title 49 Chapter and in accordance with the scope of practice permitted with licensure.
Requirements
  • Education and Licensure: Graduated from an accredited school of nursing and possess a current Practical Nurse license in good standing from the Commonwealth of Pennsylvania. A temporary practice permit is required for nurses without a Pennsylvania license.
  • Skills and Experience: Demonstrated management, communication, and observation skills with an interest in geriatrics. Working knowledge of computers and Microsoft Word is required.
