Your Role

Position Overview:

The RN Case Manager plays a crucial role in facilitating patient care throughout the healthcare continuum by effectively coordinating resources to help patients attain optimal health outcomes, ensuring access to necessary services, and utilizing resources appropriately while respecting the patient's autonomy. This position holds the primary responsibility for guaranteeing that care is delivered at the correct level based on medical necessity, while also assessing patients for transition needs to encourage timely care progression, safe discharges, and the prevention of avoidable readmissions. The role integrates national standards for case management services, which include:
  • Utilization Management to support medical necessity and prevent denials
  • Transition Management to ensure appropriate lengths of stay, prevent readmissions, and enhance patient satisfaction
  • Care Coordination to demonstrate efficiency in throughput while ensuring care is sequenced correctly and delivered at the appropriate level
  • Adherence to state and federal regulations, TJC accreditation standards, and institutional policies
  • Providing education to physicians, patients, families, and caregivers
The responsibilities of this role encompass the following activities: a) conducting accurate medical necessity screenings and submitting for Physician Advisor review, b) coordinating care, c) assessing and reassessing transition planning, d) implementing or overseeing the transition plan, e) leading and facilitating multidisciplinary patient care meetings, f) managing concurrent disputes, g) making appropriate referrals to other departments, h) identifying and referring complex cases to Social Work Services, i) communicating with patients and families regarding the care plan, j) collaborating with physicians, office staff, and ancillary departments, k) leading and facilitating Complex Case Reviews, l) ensuring patient education is completed to support post-acute needs, m) maintaining timely and concise documentation in the Case Management system, n) keeping accurate patient demographic and insurance information, o) identifying and documenting potentially avoidable days, p) reporting on over and underutilization, q) and other assigned duties.

Required Qualifications:
  • An active Registered Nurse license with a minimum of two years of acute hospital patient care experience is required. A Bachelor of Science in Nursing (BSN) is preferred. Accreditation as a Case Manager (ACM) is also preferred, along with experience in acute hospital case management.
  • 1-2 years of prior experience in case management is required.
  • Essential skills include strong organizational abilities, excellent verbal and written communication skills, the capacity to lead and coordinate activities among diverse groups in a fast-paced environment, critical thinking, problem-solving skills, and proficiency in computer usage.

    Ability to establish and maintain effective working relationships across the health system.

    Ability to maintain a work pace that aligns with workload demands.

    Demonstrated customer service skills appropriate for the role.

    Excellent written and verbal communication skills in English.

    Possession of the skills, knowledge, and abilities essential for successful performance in assigned duties.
