As a Community Engagement Nurse Coordinator, you will play a pivotal role in executing Hackensack Meridian Health's Community Outreach Wellness Plan, addressing vital community health needs while aligning with our strategic goals and centers of excellence.


This role involves significant community travel, requiring flexibility in your schedule as activities may vary weekly.



Responsibilities:
A typical day for a Community Engagement Nurse Coordinator at Hackensack Meridian Health includes:

1. Implementing outreach initiatives as outlined in the community outreach strategy.

2. Ensuring that all logistical arrangements for health screenings, fairs, educational programs, and special events are executed efficiently and accurately.


3. Representing the Hackensack Meridian Health Community Outreach department on relevant boards, coalitions, and committees that align with our community benefit objectives.

4. Participating in committees and boards as directed by management, providing updates and follow-up information as necessary.


5. Building and maintaining relationships with key community stakeholders who share our commitment to improving health outcomes in the communities we serve.

6. Coordinating nursing and health promotion services based on community needs and identifying at-risk populations.

7. Accurately collecting and entering data into outreach databases or other systems as required, and submitting monthly reports summarizing outreach activities and outcomes.


8. Collaborating with nurses, health educators, community members, and other healthcare professionals to schedule health-related activities.

9. Organizing and scheduling health education and screening programs with other medical professionals.

10. Gathering and disseminating community health information relevant to assigned areas, while staying informed about public health trends.


11. Communicating effectively with health professionals, community members, and organizations.


12. Supporting research initiatives and applying findings to enhance evidence-based care.


13. Developing and evaluating a personal professional development plan.


14. Providing orientation and training for new nursing staff.

15. Referring individuals with abnormal screening results to appropriate healthcare providers.

16. Identifying socioeconomic factors affecting health and making referrals to suitable agencies.


17. Administering social determinant of health surveys at community events and facilitating referrals as needed.


18. Delivering health promotion services in various settings, which may involve physical activities such as lifting and moving equipment.

19. Completing annual departmental competencies and required training modules on time.

20. Performing other duties and projects as assigned, while adhering to organizational competencies and standards of behavior.

Qualifications:

Required Education, Knowledge, Skills, and Abilities:
Graduation from an accredited School of Nursing recognized by the NJ State Board of Nursing.

A minimum of 3 to 5 years of nursing experience.

Willingness and ability to travel.

Strong written and verbal communication skills.

Proficient in computer applications, including Microsoft Office and Google Suite.

Preferred Education, Knowledge, Skills, and Abilities:
Bachelor of Science in Nursing (BSN).

Bilingual proficiency.

Prior experience in community health.

Licenses and Certifications Required:
Current valid NJ State Professional Registered Nurse's license.

Maintains a current NJ driver's license.

Annual CPR certification.
