Job Opportunity

Apicha Community Health Center is seeking a dedicated and compassionate Patient Engagement Specialist to join our team in Manhattan. As a key member of our healthcare team, you will play a vital role in enhancing patient engagement and improving health outcomes for our underserved community.

Key Responsibilities:

  • Utilize referrals and CRM tools to increase patient engagement and retention.
  • Provide exceptional customer service through inbound and outbound calls, ensuring seamless communication with patients and healthcare organizations.
  • Collaborate with cross-functional teams to develop and implement effective patient engagement strategies.
  • Manage call results data and scheduling to ensure timely and efficient patient care.

Requirements:

  • High school diploma or equivalent required; college education preferred.
  • Bilingual (Spanish or Mandarin) preferred.
  • Proficiency in using computer systems and software, including eClinicalWorks, Salesforce, and Outlook.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong time management and organizational skills, with attention to detail and ability to multitask.
