Job Summary

RiseBoro Community Partnership is seeking a skilled Case Manager to join our Homebase Program team. As a key member of our team, you will provide housing assistance and holistic case management services to program participants, helping them achieve long-term housing stability.

Key Responsibilities
  • Coordinate services and activities to provide program participants with necessary tools to sustain their housing and prevent homelessness.
  • Work with clients, household members, supervisors, and supplementary staff to execute the service plan.
  • Establish goals and actions that address the immediate housing crisis and underlying causes of the crisis.
  • Monitor and evaluate program participants' progress on the implementation of their service plan.
  • Confer with supervisors and program directors to determine the necessity of financial subsidies.
  • Prepare documentation for in-house subsidy applications on behalf of clients.
  • Provide referrals for clients to access community supports, public benefits, and entitlements.
  • Maintain accurate record keeping via case files and designated computer databases.
  • Engage in the arrangement, coordination, monitoring, and delivery of services to Homebase program participants.
  • Ensure program participants' rights are protected.
Requirements
  • Bilingual English/Spanish preferred.
  • Experience working in non-profit organizations and commitment to social justice.
  • Excellent communication skills and ability to advocate.
  • Ability to work independently and collaborate with a team.
  • Entrepreneurial spirit, ability to problem-solve, and build relationships.
Education and Experience

Associate Degree required, plus three years of experience; Bachelor's degree preferred.
