**About the Opportunity:**
SaVida Health, a private equity-backed healthcare company, provides outpatient opiate and alcohol addiction treatment services. We are seeking a Care Coordinator to join our team in Bangor, ME.
**Responsibilities:**
- Conduct psychosocial assessments as needed
- Develop strengths-based treatment plans per established guidelines
- Perform care coordination with outside providers
- Assist with patient health promotion and wellness as requested
**Requirements:**
- Bachelor's Degree in counseling, social work, or other allied mental health field
- One-year experience working in the substance abuse treatment field
- MHRTc license required
**Estimated Salary Range:** $55,000 - $65,000 per year.
