Job Summary

We are seeking a skilled Community Rehabilitation Specialist to join our team at the University of New Mexico - Hospitals. As a key member of our Behavioral and Mental Health department, you will play a critical role in coordinating and providing services and resources to individuals and families promoting rehabilitation, recovery, and resiliency.

About the Role

The ideal candidate will have a Bachelor's degree in a related field and at least 1 year of direct experience in a similar position. You will work closely with multidisciplinary teams and community resource providers to assist patients in meeting their treatment goals. Your responsibilities will include facilitating the development of skills in activities of daily living, interpersonal coping, socialization, and community functioning.

Key Responsibilities
  • Coordinate and provide services and resources to promote rehabilitation, recovery, and resiliency
  • Identify and address individual strengths and barriers to attaining self-identified goals
  • Assess, support, and intervene in crisis situations using individual crisis management plans
  • Maintain comprehensive tracking of client activities and coordinate post-discharge follow-up contact as needed
