Job Overview
SUNY College of Optometry's University Eye Center seeks a skilled Licensed Master Social Worker to manage the social service department in midtown Manhattan. As a member of an interdisciplinary care team, you will oversee coordination of behavioral health services for patients with diverse needs. This is a full-time FLSA Exempt position, not eligible for overtime provisions.

Responsibilities
This role involves providing integrated behavioral health services, including bio-psychosocial-medical assessments, care coordination, and referrals to social services. Key responsibilities include:
  • Conducting short-term and crisis interventions for individuals and families.
  • Organizing and facilitating groups for patients.
  • Collaborating with optometrists to provide comprehensive care.
  • Developing care coordination plans for patients as needed.
  • Supervising social work student interns.
  • Tracking and following up with patients to ensure compliance and access to benefits.
  • Educating medical staff on psychosocial issues related to patient care.

Requirements
To be successful in this role, you must have a Master's Degree in Social Work (LMSW) and at least four years of experience in healthcare or behavioral health settings. Required qualifications also include experience in care coordination, interdisciplinary collaboration, and negotiating systems. A Licensed Clinical Social Worker (LCSW) certification is preferred, along with bilingual English/Spanish language skills.

Compensation
The salary range for this position is $75,000 to $80,000 per year. Additionally, you will receive a generous benefit package, including 15 days of annual leave and 13 paid holidays per year. The SUNY College of Optometry is a qualifying employer in the federal Public Service Loan Forgiveness Program.
