Social Services Specialist I

Sea Mar Community Health Centers is seeking a highly skilled and compassionate Social Services Specialist I to join our team. As a key member of our MSS Department, you will be responsible for conducting comprehensive integrated clinical psychosocial assessments, developing intervention plans, and providing brief treatment to clients. Your expertise will help us deliver high-quality, patient-centered care to our diverse community.
Key Responsibilities- Conduct thorough psychosocial assessments to identify client needs and develop personalized care plans.
- Provide brief treatment and interventions to support client goals and outcomes.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and transitions.
- Develop and maintain relationships with community resources and services to support client needs.
- Document client services and progress in our electronic health records.
- Bachelor's degree in a related field and current certification as a counselor or chemical dependency counselor.
- Experience working with underserved populations, including those with limited English proficiency, homelessness, and migrant and seasonal workers.
- Knowledge of evidence-based screening tools and ability to use them to inform care decisions.
- Strong communication and interpersonal skills, with ability to work effectively with diverse stakeholders.
- Proficiency in electronic health record systems and Microsoft Office applications.
