Role Summary
The Patient Care Coordinator is tasked with overseeing healthcare decisions through a structured approach to ensure treatment plans enhance quality and outcomes. This role involves the coordination of care throughout the healthcare continuum, promoting cost-effective practices within established timelines, ensuring appropriate payment for hospital services in line with patient-related utilization management criteria, and implementing safe and suitable discharge plans. The Coordinator will assess the social work needs of patients and provide necessary interventions as part of the discharge planning process. Core functions include:
- Clinical Interventions and Discharge Planning Management
- Financial Oversight
- Payer and Referral Management
Essential Qualifications
- Current licensure in Social Work or a related clinical discipline (RN preferred), as per state regulations for case management and/or discharge planning.
Preferred Experience
- Prior experience in Case Management and Discharge Planning is advantageous.
- CCM Certification is preferred.
