Clinical Care Liaison Job Summary

We are seeking a highly skilled Clinical Care Liaison to join our team at Lifespan. As a Clinical Care Liaison, you will play a critical role in enhancing care accessibility within our health system.

Key Responsibilities:
  • Collaborate with the EpiCenter team to facilitate communication between practitioners at referring institutions and within Lifespan to coordinate care and services.
  • Act as the primary point of contact for clinical and administrative coordination, adhering to established departmental policies and procedures.
  • Establish and maintain communication channels using state-of-the-art communication equipment to facilitate real-time consultations, referrals, and patient transportation.
  • Coordinate with internal and external practitioners to secure and schedule clinic appointments for patients being discharged from a Lifespan facility based on the patient's identified needs.
  • Complete all required documentation with emphasis on compliance and clarity within designated time frames.
  • Collect and coordinate the dissemination of patient demographic and health insurance provider information to patient registration and business office.
  • Initiate patient registration process utilizing bed control portal and established protocols.
  • Dispatch LifePACT team as directed by Transfer and Access Coordinators and maintain communication via radio/phone with LifePACT team.
  • Track and document all required response times.
  • Work in collaboration with case managers and community paramedics to coordinate and schedule community paramedicine visits.
  • Serve as liaison between community paramedics in the field and the necessary internal services during home visits.
  • Monitor and oversee the Roundtrip system for scheduling ambulance services.
  • Assist in the coordination of ambulance needs for the Lifespan system and resolve any issues that may arise in real time with ambulance services.
  • Effectively communicate relevant information to managers and medical directors and proactively raise awareness of issues that require escalation.
  • Ensure proper functioning of all communication and support equipment.
  • Participate in community outreach and in-house educational efforts such as grand rounds and departmental staff meetings to promote awareness and education.
  • Meet and maintain mandatory safety education and clinical competencies based on pre-determined credentialing standards.
  • Maintain collaborative relationships with team members, other members of the healthcare system, and outside agencies.
  • Utilize effective interpersonal skills to identify and resolve conflicts, reduce communication barriers, and facilitate group effectiveness.
  • Participate in Quality Assurance activities both in organized meetings and through measurement of operational and clinical quality indicators for the department.
  • Participate and assist in the orientation of new employees as assigned.
Requirements:
  • Current licensure to practice as a medical assistant or emergency medical technician (EMT) in the State of Rhode Island.
  • Successful completion of any associated credentialing specific to Referral Care Coordinator.
  • Minimum of 1 year of current experience as a medical assistant or emergency medical technician.
  • Prior experience within hospital and/or EMS environment is preferred.
  • Strong computer skills with proficiency in Microsoft Excel, Outlook, PowerPoint, Access, and Word.
  • Proficiency in operating transmission equipment including two-way radios.
  • Excellent verbal and written communication skills to effectively collaborate with transport staff, physicians, and other hospital personnel and patients.
  • Demonstrated ability to provide exceptional customer service and exhibit positive customer service behaviors.
  • Must be flexible, highly organized, and able to work in a fast-paced environment.
