Job Title: Home Health Physical Therapist

Job Summary:
At Residential Home Health and Hospice, we're seeking a skilled Physical Therapist to join our care team. As a Home Health Physical Therapist, you will collaborate with our team to provide patient care plans and deliver exceptional care to our patients in the Kansas City Metropolitan area and surrounding areas.

Key Responsibilities:
Develop and implement patient care plans in collaboration with our care team
Provide physical therapy services to patients in their homes
Collaborate with our team to ensure seamless care delivery
Maintain accurate records and documentation

Requirements:
Bachelor's degree or Master's degree in Physical Therapy from an accredited college
Licensed Physical Therapist issued by the state in which you will be working
Minimum of one year healthcare experience
Current driver's license and ability to spend ~20% of your day driving to/from patient locations
A commitment to consistently meet critical deadlines for charting
The skills needed to self-manage your time and schedule
Demonstrated experience with tablets, mobile phones and EMR software
