About the Role

HCA Midwest Healthcare at Home is seeking a dedicated Physical Therapist to join our team. As a Physical Therapist, you will provide high-quality, patient-centered care to patients in their homes, working closely with our multidisciplinary team to develop and implement individualized care plans.

Key Responsibilities

  • Conduct thorough patient assessments and develop comprehensive care plans to achieve patient goals.
  • Provide skilled physical therapy interventions to patients in their homes, using evidence-based practices and techniques.
  • Collaborate with the patient care team to ensure seamless care transitions and optimal patient outcomes.
  • Communicate effectively with patients, families, and caregivers to educate them on therapy techniques and promote patient engagement.
  • Prioritize caseload and coordinate daily staffing needs to ensure efficient and effective care delivery.

Requirements

  • Licensed Physical Therapist with a strong clinical background and excellent communication skills.
  • Valid driver's license and reliable transportation.
  • Bachelor's degree in Physical Therapy or related field.
