Job Summary

Venture Forthe is a leading home health care agency in Western New York, dedicated to empowering individuals to make their own choices and live independently. We are seeking a skilled Outreach Coordinator to join our team in the Buffalo/Niagara Area.

As an Outreach Coordinator, you will be responsible for identifying potential clients and developing relationships with community referral sources. This involves meeting with clients and families in the community and in their homes, making medical/social referrals, and securing necessary documentation.

Key Responsibilities:

  • Develop and maintain relationships with community referral sources, including hospitals, doctor's offices, nursing homes, and community service agencies.
  • Serve as a liaison in matters relating to referral source satisfaction and needs determination.
  • Attend events, coalition meetings, and accept leadership roles in various community boards.
  • Identify and source opportunities to promote Venture Forthe within targeted communities and audiences.
  • Other duties as assigned.

Requirements:

  • Bachelor's/Associates Degree preferred or 2 years direct experience in Health and Human services in lieu of Bachelor's/Associates Degree.
  • Experience working with, and knowledge of Medicaid Waiver Programs is preferred.
  • Seasoned professional with a minimum of 2-3 years of healthcare experience in marketing and servicing referral sources is preferred.
  • Ability to effectively prepare and present company presentations to a variety of audiences.
  • Expert communication and interpersonal skills.
  • Possess exceptional organization skills, self-directed and possess an entrepreneurial spirit.
  • Competent knowledge base of healthcare and its applications in home care. Have experience working in healthcare and have existing and/or the potential to develop industry relationships is preferred.
  • The position covers the greater Rochester area, including surrounding areas. Expected travel up to 80%.

Work Environment:

The Outreach Coordinator will work in an office and community setting, with occasional travel required. The ideal candidate will possess a strong understanding of healthcare and its applications in home care, as well as excellent communication and interpersonal skills.
