The Community Health Worker will assist patients in accessing health resources and services, support disease self-management, and coordinate culturally appropriate care and services for our patient populations.

This position is part of a program that helps individuals and communities recover from natural disasters through community outreach, access to mental health services, and building community readiness and resilience.

Key Responsibilities

  • Represent the program in the community and work with individuals, families, groups, and community partners to provide outreach, public education, and referrals as needed.
  • Assist patients in accessing health resources and services, including mental health services.
  • Support disease self-management and coordinate culturally appropriate care and services for patient populations.
  • Participate in community outreach and engagement activities to promote health and wellness.
  • Collaborate with healthcare providers and community organizations to ensure seamless care coordination.

Requirements

  • Bachelor's degree in a related field, such as public health, social work, or a related field.
  • Valid Maine driver's license and a personal vehicle available for work use.
  • Experience in outreach, customer service, social service, or a related field.
  • Fluency in English and any languages specific to the communities being served.
  • Excellent oral and written communication skills.
  • Ability to exercise judgment in the application of professional services.
