Job Overview

Position Summary

A licensed physical therapist is responsible for delivering services aimed at restoring functionality, enhancing mobility, alleviating discomfort, and preventing or minimizing permanent physical impairments in patients dealing with injuries or medical conditions.

Key Responsibilities

  • Develops and implements a personalized treatment strategy with specific rehabilitation objectives based on the patient's capabilities and tolerance, under the guidance of a physician.
  • Evaluates patient requirements; plans, assesses, and adjusts care to achieve the objectives of physical therapy interventions.
  • Collaborates with interdisciplinary teams to establish and assess collective goals for each patient.
  • Sets practical and attainable objectives for patients.
  • Maintains accurate documentation of patients' conditions and progress in accordance with state and federal regulations.
  • Instructs patients and their families on suitable therapy techniques.
  • Enhances the functionality and skills of patients by overseeing the evaluation of all residents.
  • Communicates the status and needs of patients to the patient, family members, and other relevant professionals.
  • Ensures consistent punctuality and attendance for assigned shifts.

Additional Responsibilities

  • Complies with Minimum Data Set (MDS) protocols and effective care planning.
  • Provides guidance and clinical oversight to Physical Therapist Assistants (PTAs) working within the facility.
  • Performs other duties as required.

Qualifications

  • Ability to uphold confidentiality.
  • Understanding of HIPAA regulations.
  • General knowledge of managed care principles.
  • Familiarity with state and federal regulations regarding therapy delivery and documentation in skilled nursing environments.

Education

Bachelor's, Master's, or Doctorate Degree from an accredited Physical Therapy program.

Licenses/Certification

Current Physical Therapist license in the relevant state.

Current CPR certification.

Experience

Preferred six months of experience in a long-term care setting.
