Job Title: SSVF Case Manager

Join The Salvation Army USA Central Territory as a SSVF Case Manager and make a meaningful difference in the lives of veterans and their families.

About the Role:

We are seeking a highly motivated and compassionate individual to provide critical time intervention and strength-based case management services to veterans and their families. As a SSVF Case Manager, you will work closely with our team to develop and implement individualized case plans, provide linkages to community services, and support veterans in achieving sustainable permanent housing.

Key Responsibilities:
  • Develop and implement individualized case plans with veterans and their families
  • Provide linkages to community services, including housing, employment, and healthcare
  • Conduct regular home visits and meetings with veterans and their families
  • Assist veterans in filing for SSDI/SSI benefits through SSA's SOAR certification program
  • Provide education and support on life skills, such as resiliency, tenant education, and financial literacy
  • Collaborate with community partners, including VA HUD-VASH and Salvation Army personnel
Requirements:
  • Bachelor's degree in human services, social work, or a closely related field
  • One year of direct, full-time case management experience working with the homeless population and/or working with veterans
  • Strong knowledge of VA programs and guidelines
  • Excellent written and verbal communication skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
