Job Title: Case Manager

Job Summary:

Rosecrance Inc. is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide mental health rehabilitation services and support to youth and families, helping them achieve rehabilitative, resiliency, and recovery goals.

Key Responsibilities:

  • Provide case management services, crisis intervention, skills training, and documentation within required time frames.
  • Conduct individual, family, and group counseling sessions.
  • Develop and implement treatment plans to meet the needs of clients and their families.
  • Collaborate with the Clinical Team to ensure continuity of care and effective service delivery.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Maintain accurate and timely documentation of client services and progress.
  • Communicate effectively with clients, families, and staff to ensure seamless service delivery.

Requirements:

  • Bachelor's Degree in a related field (e.g., social work, psychology, counseling).
  • Minimum two years' experience in mental health services.
  • MHP designation or equivalent.
  • Excellent written and verbal communication skills.
  • Strong organizational and time management skills.
  • Ability to work independently and as part of a team.
