About the Position
We are seeking a skilled Social Worker to join our team. As a Social Worker, you will provide direct social service and counseling to patients and their families, assist in locating appropriate resources for post-discharge care, and participate in a multidisciplinary team to coordinate the client's plan. You will also cross-train with team members to serve as a backup resource, assess and identify cases for intervention, prioritize goals and develop plans based on client needs, and monitor and evaluate the outcome of the client's plan. Additionally, you will collaborate with third-party payors and internal case management/disease management departments as appropriate for discharge planning needs.
Requirements
To be successful in this role, you will need a Bachelor's degree or Master's degree, current licensure by the Texas State Board of Social Work, and LMSW, LCSW, or LPC preferred. You will also need BLS issued by the American Heart Association and at least one year of social work experience, preferably in acute care.
