Key Responsibilities:

  • Communicate effectively with patients, families, and healthcare providers to ensure timely and efficient care delivery.
  • Collaborate with multidisciplinary teams to address patient concerns and promote optimal health outcomes.
  • Document patient information accurately and thoroughly to support quality care and compliance.

Requirements

To be successful in this role, you must have a graduate degree in nursing from an accredited program and current licensure as a Registered Nurse. Experience in public health, home care, or hospice nursing is highly valued. A competitive salary range for this position is $ $85,000 - $110,000 per year, depending on experience and qualifications.
