Job Title: Patient Care Coordinator

Comprehensive Community Health Centers Inc. is seeking a highly skilled Patient Care Coordinator to join our team.

Job Summary

The Patient Care Coordinator will be responsible for providing exceptional patient experience, ensuring seamless communication between patients, providers, and staff. This role requires strong organizational skills, attention to detail, and the ability to work in a fast-paced environment.

Key Responsibilities
  • Monitor patient/provider schedules, ensuring timely and efficient communication.
  • Manage patient registration and lobby areas, maintaining a clean and organized environment.
  • Provide exceptional patient experience, greeting and signing in patients, and ensuring their needs are met.
  • Verify and update patient information in the medical record, ensuring accuracy and completeness.
  • Collect co-pays, deductibles, and payments, and obtain and verify patient insurance information.
  • Prepare welcome packets for future patients and register patient data through the PM system.
  • Answer and screen telephone calls, take messages, and provide information to patients and staff.
  • Sort and deliver mail to the Office Manager and attend meetings as required.
  • Prepare daily batches for the business office and notify the Office Manager of any issues or concerns.
Requirements
  • High School Diploma or GED
  • Two years of experience in a medical office setting, preferably with a medical assistant certification.
  • Strong computer skills, including experience with word processing and electronic medical records.
  • Bilingual in Armenian/English is required.

Physical demands of the position include prolonged sitting, some bending, lifting, stooping, and stretching. Good eye-hand coordination and manual dexterity are required to operate a computer, photocopy machine, telephone, and other office equipment.
