Care Coordinator I Bilingual

We are seeking a highly skilled Care Coordinator I to provide case management, mental health, and substance use services to individuals with a history of persistent mental illness and/or substance use. The ideal candidate will have a strong background in social services, mental health, and/or substance use, and be fluent in Armenian.
Key Responsibilities- Assist clients in locating and accessing healthcare services, community support groups, and resources.
- Assess clients' needs and functioning, and present cases in treatment team meetings.
- Act as a client advocate, interfacing with housing and support agencies, and developing and maintaining rapport with community agencies.
- Provide individual and group supportive counseling, and psycho-education to assist clients in coping with mental illness and/or substance use.
- Provide crisis intervention and conflict resolution services as needed.
- Interface with clinical and medical staff to coordinate client care.
- Meet Agency client service expectations, and monitor and maintain documentation of client services.
- Fluency in Armenian.
- A minimum of two years of related experience or a bachelor's degree.
- Previous experience working in a social service/mental health environment, preferably with mental health and/or substance use clients.
- Current California driver's license, car insurance, and a driving record acceptable to the Agency's insurance carrier.
- Knowledge of Agency policies and procedures, HIPAA, and county, state, and federal regulations relating to service programs for individuals and families with a history of mental illness and/or substance use.
