Role Summary

We are seeking a skilled Care Coordinator to join our team at Royal Palms Post Acute. As a key member of our admissions process, you will play a vital role in ensuring a seamless transition for new residents.

Key Responsibilities

  • Assist in the admissions process, working closely with our Care Transition function to ensure a smooth integration of new residents.
  • Receive and review inquiries regarding our facility, extracting relevant information and completing necessary documentation.
  • Make arrangements with discharge planners, families, and physicians to facilitate admission to our facility.
  • Prepare admissions paperwork for signatures prior to the responsible party's arrival.
  • Assist in the resident admission orientation program, including placement of admission materials, room inspection, and resident escort.
  • Review and obtain signatures on all necessary forms within 24 hours of admission.
  • Verify identity and financial status of residents, obtaining necessary cards and verifying entitlement to coverage.
  • Provide residents with admission information packets, reviewing and obtaining signatures as necessary.

Requirements

  • Previous experience in the healthcare field is preferred.
  • A high school diploma or equivalent is required.
  • A commitment to delivering high-quality, person-centered care with dignity, respect, compassion, and integrity is essential.
  • The ability to represent our organization in an ethical and professional manner is crucial.
  • Excellent communication and interpersonal skills are necessary for interacting with residents, family members, visitors, and other stakeholders.
